Pairc Medical Practice

 
Welcome to the Pairc Medical Practice
 
The Pairc Medical Practice is a small, traditional, family orientated medical practice based in the Pairc district on the Isle of Lewis. Our aim is to provide quality health care for all our patients. The practice takes patients from within the area of Keose to the north, Marvig to the east, Orinsay to the south and Balallan and Arivruaich to the west.
 
 
Registration
 
Patients may register with the practice by visiting the surgery in Gravir during opening hours. You will be asked to complete a registration form for each person registering with the practice. You do not need your medical card to register.
 
 
Surgery Opening Hours
 
The surgery premises are open as follows:
 
Monday:       9am – 1.30pm
Tuesday:      9am – 6.00pm
Wednesday: 9am – 1.30pm
Thursday:      9am – 1.30pm
Friday:           9am – 6.00pm
 
Please note that apart from public holidays and at weekends the doctor on duty is always available between the hours of 8am and 6pm regardless of whether the surgery premises are open or closed. Patients should phone 880272 to contact the doctor on duty.
 
 
Telephone access 
 
The doctor on duty is available to take calls between 10.45 and 11.15 every morning. Alternatively you may leave a message and the doctor will phone you back.
 
 
Out of Hours
 
Between the hours of 6pm and 8am, and at weekends and public holidays the practice is covered by NHS 24. This can be accessed either by phoning the surgery telephone number 01851880272 or by phoning NHS24 direct on 08454 24 24 24.
You can also get advice from NHS24 either by phoning the above telephone number or on the internet at www.nhs24.com.
 
 
 
Consulting Times
 
The doctor consults as follows:
 
Monday         9.30am – 10.30am
Tuesday        9.30am - 10.30am         4pm – 5.45pm
Wednesday 9.30am – 10.30am         
Thursday      9.30am – 10.30am
 Friday         9.30am - 10.30am          4pm – 5.45pm
  
All surgeries are by appointment. Same day appointments are usually available. Please phone before 9.30am that day. Patients with an urgent problem or emergency will be seen as a priority throughout the day and patients are urged to phone the surgery as usual. Patients may also book appointments online. Please ask at reception to register for this.
 
 
Extended Hours Access
 
Pairc Medical Practice offers extended hours access for patients. On the third Tuesday of every month there is a surgery between 6pm and 7pm. Please note that appointments must be made in advance of this surgery . The last appointment time is 6.45pm. A list of the dates when this is provided is in the reception area at the surgery.
Patients should note that NHS24 will still be providing Out of Hours cover from 6pm on this day and as such patients will not be able to contact the surgery by phone during this time.
 
 
Special Services and Clinics
 
All doctors carry out all General Medical Services along with the following:
 
Additional Services: Minor Surgery, Cervical Cytology, Contraceptive Services, Sexual Health Services which includes screening and treatment for sexually transmitted infections, Maternity Services, Child Health Surveillance, Childhood and Adult Vaccinations & Immunisations
 
Enhanced Services: Access to General Medical Services, Near Patient Testing, Anti-coagulation monitoring, Influenza & Pneumococcal vaccinations, Minor injury (including accident & emergency), Childhood immunisations, Extended Hours access.
 
Clinics: Diabetic, Asthma, Dietician, Podiatry, Well        person & Specialist screening clinics
Please note that appointments for all of the above are available during our extended hours access surgery on the third Tuesday of the month.
Our contract to provide the above services is held with the Western Isles Health Board.
If you have not been seen within the last 3 years or are over 75 and would like to consult a doctor please telephone the surgery to make an appointment.
 
 
Practice Nurse
 
The practice nurse deals with dressings, blood pressure monitoring, blood tests etc. The practice nurse also sees patients with chronic conditions e.g asthma, diabetes and is available for advice on other aspects of your health including Family Planning and Sexual Health Services.Please phone the surgery if you wish to make an appointment. You may also book an appointment online if you are registered for this.
 
 
Repeat Prescriptions
 
This is a dispensing practice. Repeat prescriptions must be ordered from the surgery before 10.30am and at least one working day before you need to pick them up. Please use the slip provided in your prescription bag wherever possible to order your repeat prescription. You can either post this to us or hand it in to the dispensary. For those patients who are unable to do this we can take orders for repeat prescriptions over the phone. However, patients will be asked to list each item they require to the receptionist. Home Visits
 
Home visits requested before 10.30am will normally be seen the same day. If you can indicate to the surgery staff something of the nature of your problem then the doctor may be able to bring the required treatment with them. If you are too ill to attend the surgery a doctor can visit you at home.

The surgery staff are here to help you. All enquiries regarding consulting times, repeat prescriptions, home visits, hospital transport, etc. should be directed to them first.
A member of staff will be on duty during the hours that the surgery premises are open.

The doctor on duty is employed by Western Isles NHS Board.
 
 
District Nurses and Health Visitors
 
The Western Isles NHS Board employs district nurses and health visitors who work with the practice. The district nurses can be contacted on 01851 860471, and the health visitor can be contacted on 01851 703545.
 
 
Ambulance & Hospital Car Transport
 
Please phone 01463 732450 to request hospital transport. If there is a bus service and you are able to use it, or you have the use of a family car, then it is unlikely that transport will be provided by the Scottish Ambulance Service.
 
 
Discharge from Hospital
 
To help us give you better care please telephone the surgery when you are discharged from hospital as the doctor is not automatically told that you are home. This is very important if you have been given new medication in hospital because the surgery may not have these new drugs in stock and may have to order them in specially.
 
 
Conduct
 

 

 
It is recognised that coming to see the doctor may be at a time of stress. However, the practice has a policy of zero tolerance in respect of violence or abuse, whether physical or verbal, to doctors, staff of the practice or other patients. Patients who are violent or abusive may be removed from the practice’s list of patients and will have to then be seen under special arrangements through the Western Isles NHS Board.

 

 
Practice Complaints Procedure
 
To help improve our service to you we operate a practice complaints procedure. If you would like more information about this please ask a member of staff for our leaflet. In the first instance complaints should be made to Tamsin Smith, Practice Manager. Also if you have either a comment or complaint you are very welcome to contact Christine McKee, Primary Care Manager, Western Isles NHS Board, who will do her best to help you.

Premises and Disabled Access
 

 

 
The premises comprise of a doctors consulting room with attached examination and treatment room, a practice managers office, dispensary, general office with reception area, a community room and a waiting area. There is a ramp with handrail to the surgery entrance. Entrance doors are wide enough for wheelchairs and the premises are on one level. There is a WC suitable for disabled patients. There are also three fire exits with ramps. Cars must be parked on the public road outside the surgery – please take care when getting in and out of your vehicle. 

 

 
Carers
 
The practice is able to offer carers practical advice and support. If you are a carer please let us know for our records.
 
 If you are unable to attend the surgery because of a disability you should discuss this with the doctor and other arrangements may be made. 
 
 
 
Confidentiality
 
To provide you with the care you need we hold details of your consultations, illnesses, tests, prescriptions and other treatments that have been recorded by everyone involved in your care and treatment e.g. GP, Health Visitor, Practice Nurse. This information is stored both on paper and electronically. The practice is registered under the Data Protection Act for this purpose. You are entitled to see your medical records on written request and you may take copies, though there is a charge for the copies.
Where it is necessary relevant information is exchanged with other health professionals, such as for hospital referrals, or to central systems such as health monitoring and screening. No information is however disclosed outside of the health service without your permission unless we are required to do so by law, such as for a court case. If you want any more information on this we have a more detailed leaflet available. Please ask for it at reception
 
 
PATIENTS RESPONSIBILITIES AND RIGHTS
 
You should share in the responsibility for your own health.
You are entitled to information and practical help towards healthier living, and you are entitled to expect the staff of the NHS to set a good example.
You are entitled to the highest standards of care possible for yourself, your family and friends and to be treated as a person, not a case.
You are entitled, if you want, to accurate relevant and understandable explanations of your problems and your options for treatment, and, if you wish, a second opinion.
You are entitled to be involved so far as is practical in making decision about your own care, and wherever possible given choices. 
You are entitled to know more about the standards of the hospital, community or other health services you are using.
You are entitled to expect close links between different aspects of health care.
You are entitled to expect all NHS staff to ask themselves on each occasion “How would I want to be treated if I were that person in that situation?”
 
You are entitled to access, within safeguards, to information held about you by the NHS; and to be sure that this information will be kept confidential
 
You are entitled to know how long you are likely to have to wait for an out-patient appointment.
You are entitled to know how long you may have to wait to be seen at an out-patient clinic.
You are entitled to expect that any comment you may have will be taken seriously, and that any complaints will be treated fairly and quickly.
You are entitled to readily available information about how to complain and to help and advice.
You are entitled to clear information about the specific services offered by each GP practice and the facilities available.
You are entitled to improved information about NHS services.
You have a responsibility to keep appointments or to contact the practice in sufficient time for the appointment to be re allocated. Patients who fail to keep appointments waste valuable time that other patients could use. Patients who consistently fail to keep appointments may be removed from the practice’s list of patients.
